Statements and consent

1. I declare that I am of full age and that the voice of which I hand over a sample is my own voice.
2. I agree that the contractor creates a computer model of my voice from the sample I hand over.
3. The voice model may be used only to narrate the film or films described in the order.
4. I know that the film tells viewers that a computer recreated the voice.
5. Before signing this consent, I received a separate notice about the processing of my data by Krzysztof Głuszczyk (Creatiwi AI), complying with Article 13 GDPR, and I can retain a copy. It specifies the data, the purpose and legal bases of processing, the recipients, the name of the speech synthesis provider, the countries of processing, the basis for any transfer outside the EEA and how to obtain a copy of the safeguards, the retention periods, the rules for deleting the sample and model, and my rights and how to withdraw consent. The sample and model may be used only for narration of the ordered films, by the contractor and the processors named in that notice. This consent does not cover training the provider's general models or other orders. Nor is it, in itself, consent to a transfer outside the EEA under the derogation in Article 49 GDPR. A transfer requires a separately established basis, such as an applicable adequacy decision or standard contractual clauses and any necessary additional safeguards. If I have not received this information, I do not sign the consent or hand over the sample.
6. The sample and voice model will be deleted within 30 days after the ordering party receives the film, or without undue delay if consent is withdrawn earlier. I consent to the use of my cloned voice in the films described in the order, to showing them in the ordering party's own institution, company or home, and to sharing them through their own channels. This consent has no specified end date and remains valid until withdrawn. Deleting the sample and model does not in itself mean deleting the finished film; after consent is withdrawn, the rules described below apply.
7. I freely give my explicit consent to the processing of my sample, voice model and narration that allows me to be identified, solely to create and use the films described in the order, under Article 6(1)(a) GDPR and, to the extent that biometric data is processed for the purpose of uniquely identifying me, Article 9(2)(a) GDPR. I know that not every recording or cloning of a voice constitutes such processing, and that the technology used and the scope of the data have been described in the information I received. I sign this consent personally as the adult voice owner. No one has pressured me; I may refuse or withdraw consent without a penalty. If I am an employee of the ordering party, use of my voice results from my free choice, and refusal or withdrawal of consent does not affect my employment, pay, evaluation or working conditions. I know that the ordering party may choose the author's voice or a library voice. The person signing this document on behalf of the ordering party confirms their authority to represent the ordering party and the ordering party's commitment to ensuring this free choice; their signature does not replace my signature or my consent.
8. I may withdraw my consent at any time, without giving a reason and without a fee for the withdrawal itself, by writing to orders@bipsclub.com. Withdrawal does not affect the lawfulness of earlier processing. The contractor stops further creation and use of narration with my voice, deletes the sample and model without undue delay, and instructs processors processing them on the contractor's behalf to delete them. The contractor also deletes copies of the film remaining under their control, except for necessary retention permitted by law, without further sharing. The contractor informs the ordering party and the relevant recipients of the withdrawal. The ordering party stops showing and publishing the version with my voice, removes it from their channels, deletes copies intended for further use, and informs the entities with which they shared it. The contractor takes the reasonable steps required by law concerning further copies, but cannot guarantee retrieval of downloaded files or deletion of all copies outside their control; this does not restrict my statutory rights against those holding them. The film license does not replace my consent. Withdrawal alone does not terminate the order contract; the contractor and the ordering party separately agree on replacing the voice, further performance, or termination and settlement of that contract, preserving statutory rights. If I am not the ordering party, I am not charged for the costs of changing the order or penalized for withdrawing consent.

This template covers only the voice of an adult. I do not clone the voices of children.
